COVID-19 self-reporting acknowledgement for visitors and contractors

Okanagan College is following directions and controls specified by the B.C. Ministry of Health and Provincial Health Officer, the B.C. Centre for Disease Control and Interior Health. To learn more, please visit the B.C. Government's COVID-19 website.

To ensure everyone’s safety and well-being, Okanagan College is conducting daily active screenings for potential risks of COVID-19 with all individuals before they enter Okanagan College buildings. If you fail to meet the screening, you will not be permitted on campus. It is expected that you stay home, seek medical advice, and contact your supervisor, if you are an employee, or your point of contact, if you are a student or contractor. If you are feeling unwell, you can check your symptoms online, or download the BC COVID-19 self-assessment tool.  These resources are available 24 hours a day.

By checking this box, I understand and agree that I am only permitted on campus:
  • if I have not travelled outside of Canada in the last 14 days;
  • if I am not currently ill or suffering from any COVID-19 like symptoms that include a cough, sneezing, fever, sore throat, difficulty breathing. Please refer to the BC Centre for Disease Control website for a full listing of COVID-19 like symptoms; and
  • if I have not been in close contact with someone who has been suffering from any of the above symptoms or someone who has a confirmed or probable case of COVID-19.
By checking this box, if I start experiencing COVID-19 like symptoms while on campus, I agree to immediately leave campus and not attend face-to-face classes until I have met the College’s requirements to return. Students who are unsure about symptoms should self-isolate and use the BC COVID-19 Self-Assessment Tool, call 8-1-1 or the local public health unit to seek further input, and contact a family physician or nurse practitioner to be assessed for COVID-19 and other infectious respiratory diseases.

The information collected through this form will be used by Okanagan College only for the purposes of addressing infection risk in the workplace and in the learning environment.  All personal information will be received in confidence and is collected under the authority of the Freedom of Information and Protection of Privacy Act, the College and Institutes Act, and the Workers Compensation Act and related regulations.
